- Comprehensive Child Health Profile
- Immunization Compliance & Medical Verification
- Lead Screening Verification Record
Additional Medical & Care Plans (If Applicable)
The following documents are required only if they apply to your child’s medical needs.
- Authorized Medication Administration Directive
- Individual Asthma Management & Care Plan
- Individual Allergy Response & Care Plan
- Seizure-Response Medication Authorization Plan
These documents may be uploaded through the portal or submitted separately via secure email or in person prior to your child’s first day.
