The Enrollment Readiness Dossier

A Curated Suite of Documents for Families Preparing with Intention

At Prime Montessori Academy, enrollment is both a thoughtful and ceremonial process—one that honors the child, supports the family, and upholds the Academy’s Premier Standard.

As part of this journey, certain health and compliance documents are required by the Maryland State Department of Education (MSDE) for any child who enrolls.

Because some documents require time to obtain from pediatric or healthcare offices, this dossier is offered as a courtesy to families who wish to prepare in advance. Completing these forms at this stage is optional and does not indicate enrollment.

If an enrollment invitation is extended, families who have gathered these documents in advance are often able to complete the Enrollment Compendium with greater ease.

A NOTE ON TIMING

Many families choose to begin requesting these documents now, as pediatric and healthcare offices may require several days — or even weeks — to complete state-mandated forms.

Preparing in advance supports a seamless enrollment process should an invitation be extended.


Upon enrollment, these documents become part of your child’s official Enrollment Compendium and Maryland State compliance record.

You may download or print these documents at your convenience. Please retain copies for your records and for your child’s enrollment file if an invitation is extended.
